The Acknowledgement of Personal Liability & Waiver form is a required document for all students participating in field trips. This form acknowledges the inherent risks of field trip activities and releases the school and its representatives from liability.

• The form must be submitted prior to your child’s participation in any field trip.
• If you choose not to sign this form, your child will not be eligible to participate in school field trips.
