The Field Trip Release & Consent Form is required for any student participating in field trips organized by RCA.

• The form must be submitted prior to your child’s participation in any field trip.
• If you choose not to sign this form, your child will not be eligible to participate in school field trips.

You have two options for completing and submitting this form:
